  SERVICES INCLUDE:

• A Geriatric Nursing Assessment: An assessment of the individual’s current physical, cognitive, emotional and spiritual status. This assessment also includes: current medical diagnoses, medications, treatments and a visit summary.
• A Caregiver Skills Assessment: An assessment of the Caregiver’s ability to provide safe and compassionate care.
• Identification of “Facility Acquired Conditions” – falls/trauma leading to serious injury/hospitalization, development of pressure ulcers/bedsores, Sepsis/Dehydration. To assign monetary accountability, this assessment also includes identification of those responsible for the harm.
